We know your time is valuable, we do our best to make your first visit as efficient as possible. If you take a few minutes to complete the online health questionnaire and patient information form, your patient chart will automatically be created in our system. If you prefer to do it by hand at your appointment, please arrive fifteen minutes early to fill out the paperwork. On the form, you will be asked to enter an appointment date. Please enter any valid date, we will make your appointment via phone call.
